QuickBooks Online accepts CSV: use the QuickBooks Online CSV export, which is the 4-column layout (Date, Description, Credit, Debit) QuickBooks reads without column mapping. QuickBooks Desktop uses Web Connect (.qbo): use the QuickBooks Desktop (.qbo) export and open it from File → Utilities → Import → Web Connect Files.

Every line on the statement: purchases, fees, interest, deposits, transfers, refunds and reversals, each tagged as money in or money out. Totals are checked against the statement so the import balances first time.
